In consideration of being permitted by Indoor Football Ireland Ltd t/a Dublin Indoor Football, to participate in its activities and to use its equipment and facilities, now and in the future, I hereby agree to release and forever discharge Indoor Football Ireland Ltd t/a Dublin Indoor Football, its agents, owners, members, shareholders, Directors, partners, employees, volunteers and affiliated entities and successors (the “Released parties”) as a participant, guardian and/ or supervisor of the child/ children as follows:

1.       I wish for myself and/or the child/ children, to participate in the activities organised by Indoor Football Ireland Ltd (CRN 503939) trading as Dublin Indoor Football (“the Company”) whose registered office is at The Soccer Dome, Sportslink Complex, Santry, Dublin 9 D09 V8XT.  

2.       The activities referred to, include but are not limited to indoor football, bubble-football, fitness classes, tag-rugby, Frisbee, Segway activities, party- room access,  offsite and associated activities (collectively hereinafter called “the Activity”), as organised by Dublin Indoor Football and/or a third party.

3.       I confirm that I am 18 years or older and confirm that the child/children partaking in  the activity/activities, is/are under 18 years old but 5 years’ age and/or older.

4.       If applicable, I declare that if I am not the parent or legal guardian of the child/ children, I have authority from the parent or legal guardian of the child/children to sign this Disclaimer and Injury Waiver Form. Whilst attending Dublin Indoor Football and whilst participating in the Activity, the child/children will be under my care and supervision or alternatively, I will ensure that the child/children will be placed in the care and supervision of a suitable adult with my full permission.

5.       I, as the parent/ guardian/ supervisor of the participant, acknowledge that their participation in any and all of the activities provided by the Company and third party providers, entails known and unknown risks that could result in physical or emotional injury, paralysis, death or damage to themselves their property or to third parties.

6.       I, as the parent/ guardian/ supervisor of the participant, warrant that I will comply and ensure that the participant complies with all written and oral safety instructions and advice provided by Dublin Indoor Football, at all times whilst on the premises from which the activities will be carried out. In particular, not to wear mouldies, blades, studs or to play in socks or bare feet whilst on the pitches.

7.       I acknowledge that as a participant, relevant guardian or supervisor, I am responsible for my own safety and the safety of others under my care whilst partaking in the activities of Dublin Indoor Football.

8.       The participant warrants that they comply with any age restrictions that may be in place by the Company from time to time to include the following:

·         Age Restrictions for indoor football= Age 5 +

·         Age Restrictions for bubble football= Age 8 +

9.       I acknowledge and accept that the Activity requires a moderate level of fitness and is physically testing.

10.    I confirm that I do not know of any medical condition that I or any of the child/children may suffer from, which might have the effect of making it more likely that I or any of the child/ children be involved in any incident which could result in injury to myself, the child/children or others.

11.       I acknowledge and accept that by participating in the Activity, I and/or the child/ children are exposed to the possibility of personal injury or death and/or property damage and I accept this risk on my behalf and on behalf of the child/ children.

12.       In the event of an accident involving myself and/or the child/ children, I acknowledge and accept that Indoor Football Ireland Ltd t/a Dublin Indoor Football, will not be liable for any direct or indirect loss, damage or injury arising from or in connection with my and/or the child/ children’s participation in the Activity and I hereby waive all and any claims against Dublin Indoor Football in this respect and in respect of myself and/or the child/ children.

13.       I hereby indemnify Dublin Indoor Football against all claims made by any other  person against Dublin Indoor Football in respect of any injury, loss or damage arising out of or in connection with my failure and/ or the failure of the child/ children to follow safety instructions and/or directions of Dublin Indoor Football its management and/or staff or third party.

14.       I acknowledge and accept that this Waiver may be pleaded as a bar to any action or proceedings taken by me and/ or the child/ children at any time against Dublin Indoor Football, arising out of or in connection with my and/or the child/ children’s participation in the Activity.
